Most insurance companies require the following qualifications for bariatric surgery:

  • Age 18 or older
  • Body mass index (BMI) greater than 40 or greater than 35 with Type 2 diabetes, sleep apnea, high cholesterol, GERD or other obesity-related health conditions
  • Multiple unsuccessful attempts to lose weight with diet, exercise or weight loss medications
  • Nonsmoker or willing to quit smoking at least six weeks before surgery
  • Willing to commit to a healthy, active lifestyle

If you don’t meet these qualifications, you may still qualify for bariatric surgery at Atrium Health Floyd and Harbin Clinic with a self-pay option. Our team can help you understand your options.
